CD Application License Verification Form

Sponsoring libraries should use this form to confirm their License Agreement for cd applications they wish to host on the Library Systems Office's Citrix server. The form should be as complete and accurate as possible. If necessary, use the form as a guideline when requesting information regarding your license agreement from vendors. Please print and sign the form upon completion and return, along with the cd for which the form applies, to the Library System's Office (LSO) Main Library room 314 MC 522. The Library System's Office will not host any cd application without this written commitment from the sponsoring library.

*This service is ONLY for those applications that do NOT have equivilent Web access!
* Not all cd's can be hosted using the Citrix server for technical and logistical reasons. If your vendor requires additional payment for this type of service, please bring the cd to LSO for confirmation that the the cd is mountable on our system before authorizing payment to the vendor.

The form is in .pdf format and can be viewed using Adobe Acrobat Reader. The form is interactive and you may type into the form before printing. You should save the form to a local directory before doing this, however, as any changes made to the form will be saved in the file. If you wish to keep a personal record of your verification forms, simply save the file with a unique name (ideally something that would help you to identify the application to which the form refers). Please remember to print and sign each form before returning to the Library System's Office.
